Richard,
I can tell you it works because I am doing it with a HP1020 laser
printer and a MAXNC-10 mill.
I use Eagle to layout the PCB.
There is a ULP called PCB-GCODE to generate G-CODE for the CNC mill.
Once I have the drilled PCB I use toner transfer to etch one side at a
time masking the other. To get the alignment it the ball park I insert
stick pins in a hole on two diagonal corners, then fine tune under a
bright light.
