My last board (one panel, 100 circuits[∗]) was from pcb-pool.com. 6
mil rules, no minimum qty (16 sq in minimum), 1-6 layers, and
panelization is allowed. Otherwise, it depends on your design needs -
board size, smallest hole, line/space rules, layers, etc. 4pcb has a
"bare bones" special if you don't need solder mask, batchpcb has a
TINY minimum order (er, 1 sq in) if you don't mind waiting, and olimex
has some sweet deals on standard size boards.

pcb-pool has a volume option as well, and 4pcb lets you go from a
3/$33 prototype to full volume production.
