Set Attack, Decay, Sustain Release as you would for any ADSR.
Set Slope to zero.
Set Breakpoint equal to Sustain.
By setting Slope to zero you ensure that ADBSSR becomes ADSR by making Slope zero time long (at least not discernible to the human ear). Setting Breakpoint level to Sustain level ensures that Decay decays to the Sustain level (because Breakpoint and Sustain are the same).
