In the end this is like with all electrical safety measures. You can 
have organizational safety measures like a policy not to work on powered 
equipment unless strictly necessary. But that does not mean physical 
measures like lock-out-tag-out or putting insolation over exposed pins 
are useless just because you have some policy.

Usually you want to have more than one layer of safety¹, which is why 
discussing how to phsyically deal with exposed pins is not a useless 
endeavour. Having pins insulated may also help you with unpowered 
systems as frying sensitive ICs with static charges is also harder when 
there is less stuff to touch.

But yeah:
1. If you _can_ power it off, you should
2. It is only off if you _measured_ no power
3. You should make sure you can't accidentally turn it on (unplug it)
4. Only _then_ you should start working on it

If you absolutely have to work under power for some reason (and there 
are valid reasons for doing so), you should know which bits are 
dangerous to you and the thing you are working on and you should cover 
them with something nonconductive. Could even be a piece of wood.

Not that hard.
