[sdiy] Combined output / headphone jack?

Looking at the schematics of the EMu EIV I see it has balanced outputs
using the SSM2142 balanced line driver. The main L/R outputs have both XLR
and TRS jacks, while the three pairs of SUB L/R outs just have TRS jacks.
