I began and ended my electronic career repairing cable TV line extenders and their power supplies. There was one made by a famous company that was a good design and easy to work on. There was just one slight problem: the fuse was after the bridge rectifier. This led to lots of fried transformers. These things run off of 60V square wave AC so it was a special transformer that was quite expensive.

At this time in the early to mid '80s surge protection was uncommon. We made our own little crowbar devices in-house and installed them on everything that was given a full one year warranty. One day we got a phone call from an engineer at the company asking us why the stuff we repaired didn't burn up as often.
