I highly recommend the revised hardcover edition of the following by Thomas Frederiksen:

Intuitive Operational Amplifiers: From Basics to Useful Applications (McGraw-Hill series in intuitive IC electronics)

Thomas worked by Bob Pease at National Semiconductor and designed the LM324 and the LM3900.
