How much delay can you tolerate ?  There's an android app which divides the signal into each note, including it's cents offset in fairly real time, i.e not using an FFT, then outputs to USB.  Will do what you are after far more accurately than any analogue circuit.

The answer is that I’m extracting from the full signal, not individual strings

The External Signal Processor from the Korg MS-20 is a very useful circuit worth looking at, but IIRC requires at least one multigang pot that could be difficult to get, though may be available now due to Eurorack clones.
