Mike, I think what you said is true, you have to just consider every part and what you put there (resistors included now).  Bruce Hofer also has done some work in this area and concluded that distortion got worse the smaller the part and that 1206 thick films were fine for most circuits (a "sweet spot" in price/availability) and for critical areas thin film SMD parts were preferred (but of course $$$$).

Ø Off topic but regarding SMD resistor distortion, I disagree that "In fact it is SMD resistors that have become the main source of distortion in surface mount products."  I think this is an over-generalization.
 
  
 
Nowadays there often aren’t that many capacitors in the signal path, but the number of resistors just seems to grow so the accumulative distortion from resistors begins to dominate.  Douglas Self covers resistor distortion well in his book.
