Welcome to the group!
If the voltages at the potentiometers are ok, then the only two
possibilities I see is are either the 4558 (IC14 - causing some number
overrun inside the D/A if to much voltage is sent) or (more ugly) the
YM26700 itself (IC11).
The 4558 is easy to replace, the YM26700 is quite an unobtainium
