I've used Lulu.  They're a little on the pricy side, but the physical
quality of the product is good.  Last time I looked at Amazon's service
(which I think is called CreateSpace), their requirements regarding page
numbering, strangely enough, were dealbreaking for the particular project
I was working on at the time.  I didn't look further, but had the
impression they might be worth considering for some other project.
