After searching a lot I settled on the TSH72 or TSH82 for these needs.
Output is rail-to-rail. Input includes negative rail. 70Mhz gain
bandwidth product and high slew-rate makes it good for filters. They
also have a beefy output.
Only drawback: Supply is limited to -5/+5. Or 10V if you want to run it
in single supply configuration.
