High dpi exports are essential !!

also some programs will only accept imports if everything is exported in 100 % true black (Osmond has this issue and for some imports diptrace does as well )

PCBcart are who I use for production panels - they usually do good work with low discard rate (5-10% max - panels with scratches/blemishes) but I have sometimes had to ask them to re-do part of a batch if the results were not acceptable.
Actually it is quite good of them to agree to such, as their work is more about creating fully functional PCBs rather than 100% scratch free.
I have sometimes tried other board houses but haven't been convinced to change.
