It was mentioned on a high-voltage forum that I frequent.  Apparently it 
runs from a 107 VDC nixie tube power supply.  Each note uses it's own 
relaxation oscillator made up of a neon bulb, capacitors and resistors.  No 
microcontrollers, transistors, diodes, op-amps, etc.  Copper foil is used 
for the keys, and tremolo looks like it's implemented by varying the 
pressure on some carbon-loaded anti-static foam used to play the notes.
