The discussion I had off list was, I believe, with the same person who
offered to share their scans of this supplement with me.  Based  on the
size, that's what he said he had as well.

If I am able to download the zip, I'm willing to take a crack at cleaning
the scans (I've done a lot of photoshop work over my years) and making a
PDF to send back to you.

If you'd rather I didn't, and/or don't want the person to send me the
scans, I will respect your wishes, but I'm just scratching a curiosity itch
right now regardless of how practical it is :).
