I always thought this was due to the difference between 1/8" (125mil or 3.175mm) and 3.5mm (138mil). But I recall an explanation that there really is no such thing as a 1/8" standard, even though there is a 1/4" standard. Some places like Mouser do quote 1/4" hardware as 6.35mm or something close.
I stepped away from this email to search Mouser for examples, but got sidetracked…
