[sdiy] MIDI parser

In my digital development of both poly and mono synths, I've written several MIDI parsers
(controllers) which also serve as voice managers.  In my view, it can be advantageous to have
control over the parser/controller code.  Each different way to deal with synthesis can have an
effect on what is needed from the MIDI controller/parser - to move computational load away from the
sound generation code.  So I roll my own...
