you can find virtually all sorts of approaches.
DIY one-offs built upon dev boards and add-ons (“shields”), custom PCBs with powerful DSPs and elaborate code (e.g. Synthesis Technology), cost efficient smart designs based on fast arm SOCs (Muteable Instruments), hybrids containing analogue and digital circuits, FPGA synths (e.g. Scott Gravenhorst), pics with and without DSPs (e.g. Tom Wiltshire) and so on.
YMMV of course and this list is just from the top of my head which is certainly not complete and named people may describe their work differently.
So just to give you an idea and some starting point.
