[sdiy] New FM shield for Arduino

Hi again,

no jokes but I did the engine from memories of the sound of the
OPL2 and and having my FB01 (YM2164) on the desk.

Did not had much look at the theory paper, being more focused
on getting the right ranges and curves for the user to program
the sounds.

I will read it to see what I have missed, software can be still improved.
