Worth noting that that section seems to be written on the assumption of
single-supply operation; the inputs shown as connected to ground in the
figure are really connected to the negative supply line.  The "good"
figure, as described in the text, corresponds to hooking up the amp as a
voltage follower following ground (as labelled "NO!" in the figure) in a
dual-supply system.
