[sdiy] Midi 3.5mm cable

I just made a Roman Sowa midiimplant module for my Eurorack, in a very
skinny panel, so no din. I had an adaptor for a Beatstep laying around, tip
= pin 5 and sleeve = pin 4.  That's all you need.
