Quick update:
While I haven't solved the PCBs yet, I have built a CA3080 test
circuit (the unity gain follower from the datasheets) and proven that
the ones that were on the PCBs are good.
I also proved that the 25 I bought in 2012 are all crap. :( So that
is a leading candidate for what was wrong with the breadboard version.
