The monome folks just announced aleph

http://monome.org/aleph/

No MIDI, per se, but I believe it could easily host a USB-MIDI interface
4 audio in / 4 audio out
4 buttons
4 encoders
has some kind of expansion port that might support more knobs/buttons/ 
switches
OLED (grayscale)

... also has 4 CV in / 4 CV out

I don't know if it can be sourced later for manufacturing, but it's  
$1,400.00 to consumers so it's not cheap

I think you'll find that it's probably too expensive to make a  
generic platform that does everything you list, which is why most  
products have a custom PCB tailored to the exact needs of the device.  
Or, at least I don't think everyone could agree on the feature list,  
which sort of reduces the potential market size.
