The single most effective change in your arrangement would be to put the PCB 
stack vertical instead of horizontal.  Leaving a hole above a hot component 
is better than nothing, but certainly not as effective.  A pin-finned 
heatsink on the hot component protruding through the hole on the other hand 
would likely be a better arrangement than the same in open space (without 
the second PCB).  If you really must know, build yurself a thermal dummy and 
check the temperatures for a few variants.
