This is too far off from what you'd reasonably expect from a 12V output, 
especially when no load is present.  What is the voltage after the 
rectifier, across the load caps?

If it sparked over onto the power supply only, then the most likely 
culprits, given your readings, would be the rectifier bridge and the load 
caps.  These caps are somewhat under-dimensioned anyway, so replace them 
with ones rated 100µF/35V (or even 50V if they fit physically) once you are 
sure the rectifier works correctly.  Then check the output voltages after 
the regulators again, first without load and then check that things are 
stable under load.
