There's also the ones from WQP (PJ-301BM) - get the newer ones with 
beryllium copper contacts that don't fail after a few insertions.

Oh, and when you do your PCB layout make sure there is a hole in the PCB 
to accommodate the tip of the plug, or to blow out any crud that falls 
or gets pushed in there.
