One jack: one tip, one ring, one connection back on the PCB (consider .100" MTA connectors as well; they look neat and are a good servicing and assembly aid). This also promotes start grounding, ie: minimizes noise. The ground wire running alongside the signal wire, ideally the two being twisted together, will also help with noise.
