I'm really digging deep here. If I get it wrong maybe someone else can
comment.

Are you using a PC disc or an actual replacement floppy drive?

If memory serves most floppies have strapping/jumpers/traces on them to
make them either disc 1 or disc 2 via a select line. For PC floppies
they were usually configured as disc 2. The twist in the PCs floppy
cables would then handle the disc 1/disc 2 selection. So if a PC drive
is being used it could have the select configured wrong.
