1) To use MIDI in your product: no licence, free

2) To use the "MIDI" logo - you need a licence ($$$)

3) To be assigned your own manufacturer-specific ID: annual $$$

So *only* if you need the official logo on the box *and/or* you really 
need your own SysEx Manufacturer ID -- because you can't find a way to 
use the non-specific SysEx protocols (Sample Dump and File Transfer 
spring to mind) -- then I'd say you need to pay up.  If you can avoid 
those two then I figure you don't pay a cent.
