I'm not going to confess the stupidest thing *I* have done.. but a customer
bought in a broken Frostwave Quad MDI-CV converter. It broke after he
plugged a CV output into a 1/4 jack in an organ. Why, I can't imagine..
anyway, the 1/4 jack was apparently being used as a DIY mains power
extension (!!!!) and the PCB trace from the CV socket actually blew off the
board & curled up like a watchspring.
Remarkably, and very fortunately, all that died was a 50c CMOS buffer.
But I do not know what 'protection' I should have used..
