Just a note about PICKIT 3. In MPLAB 8.43, the PICKIT 3 release notes say:

PIC18F45K20/46K20 MCU family: There is a silicon issue that prevents some 
device revisions from being programmed with "power from programmer". The 
workaround is to use "power from target" OR increase the capacitance across 
VDD, VSS (for example to 47uF.)
