Howdy Barry,
Having been through this a couple of times recently:
For a quasi-custom package like you describe, you might look at the offerings from OKW, Serpac, Pactec, Hammond and the like. They often have small plastic clamshells with flat endpieces cut from sheet stock. It costs a lot less to have those ends custom drilled & printed than it would to have a full-on custom enclosure made.
Full-custom is truly expensive - the tooling is expensive, and the minimum quantities are often steep.
You might also consider metal enclosures. I've worked with a vendor who was willing to work with me on the design, and their per-piece charges are much more reasonable than similar off the shelf Bud/Hammond boxes, and include custom drill/PEM/silkscreen. In the audio world, metal gets more respect from the consumer, too.
