Simple enough to understand and adapt to your own needs, all with one quad op-amp and it can function like an LFO as well.  I make no claim that it is the best adsr, but it does have about the lowest chip count and can be built in an hour or two using bits you already have in your junk box (I don't think it is too critical which op-amp type you use).
