If you want it perfect, it would react on both types, poly and mono.  
I've never seen a MIDI controller that sends both types at the same  
time, so your converter wouldn't get confused.
Many MIDI controllers only send mono aftertouch, so in poly mode I  
would allow for both types too.

CC1 is Modulation Wheel, which you have already covered.
Can you make it selectable from your user interface? This would be  
the most versatile solution. Yes, some CCs are used more often than  
others, but some MIDI controllers use exotic ones. The Yamaha WX5 for  
instance sends CC#80 and CC#81.
