But the fact is (and that is what Tom is implying I think) that there
is plenty of synth hardware out there (including midi-cv and quantizers)
that is perfectly in tune with 5, 6 or 8-bit DACs.  So somehow even
these old DACs can keep a perfect tune.

I very much doubt a DAC can be off -1LSB on one step and +1LSB on
the next.  I think your calcations show a theoretical worste-case
error which almost none of the DACs will actually have.
