It depends on what you mean "fit over". It should slide over without the
threaded item (a bolt for the sake of argument) needing to be twisted.
Washers are normally just a bit larger than that though, so an M6 washer
would probably be a 6.2mm hole, so it's not too difficult to get on, but
doesn't vibrate that much. The outer diameter can be anything in theory.
Note that the thread pitch changes for each bolt size, so a metric
standard thread (this is what the website that Graham pointed at is
giving the values for). There's also a metric coarse thread too (a bit
like UNC and UNF) for some of the other sizes (look for the M10 set - it
has M10x0.75 [fine], M10x1.0 [standard] and M10x1.125 [coarse]).
