I've always used this Olimex clone...

They also do an ultra-cheap RS232-only version:

Works fine for me even with the newest dsPICs (although I remember Eric
reported problems with his clone on newer dsPICs - not sure which clone
it was).
My Olimex clone can be a bit finicky with dsPICs when it comes to how
things are powered up - turning on my bench PSU at the mains switch
tends to crash the communication whereas connecting the live power lead
manually to the target board is fine. Never used a real ICD2 so no idea
if this would also have the same problem.
If I was buying another ICD now, I'd seriously consider the ICD3 as it's
much faster for programming and debugging, and generally all round
better designed for use with high speed, high capacity uProcs.
