Being ancient I can claim a very early run-in with a Microsoft product. Set the wayback machine to the early 80's. We had a DOS machine with an early version of Autocad, and lots of un-backed-up CAD files. I went to format a floppy but forgot to enter the drive letter. DOS defaulted to c:\ back then, so it merrily started formatting the hard drive. Since the directory is stored in the first tracks the format instantly made all data useless.

Backing up is a lesson I've had to learn more than once in my career. Tough lessons indeed.
