You may not get +-5V swing out of that device. In the datasheet
the gain is specified as 0.99 to 1.00 for a +-1.5V range. The
output voltage swing is specified for a +-2.5V range. Also the
min range spec for +-2.5V is +-2.2V which is a gain of about
0.88. Maybe raising the supply voltage and reducing the output
load would help.
