Perhaps you are not looking at the output correctly and you
have inadvertently created the BOG (bottom-octave generator) ???

After getting a handle on the oscillator issue (thanks again), I went to program the Xilinx part. No joy. I verified the original board still programs, then I took a closer look. It turns out that pesky VccAux line, the one that I should have paid better attention to, powers the JTAG section of the device. And of course I have it tied to the wrong voltage. 

Snarking fardwards!

Just so we're clear: the guy who always lectures others on reading the data sheet has failed to read the data sheet. I'm not a hypocrite, i'm just stupid.

So now I get to veeeery carefully lift a pin on a 0.5mm pitch part and green wire it. With luck I can clear all the problems by hacking this rev and just need one more board turn. 

sigh.
