Whenever you change something, you need to have both the board and the schematics open.  If you do this, they will keep in synch as far as this is possible.  If you edited the schematic without the board open I think an ERC should spot the differences, but you need to manually correct them.  If the differences are slight (pinswap or something like this) it is usually better to just rip a few traces and re-route incrementally.

Execute "ripup *" in the command line or click ripup, then the green traffic light symbol.
