That's the CS-15, not the CS-15D. If somebody finds it, I'd love to  
have a copy too.

I have a CS-15D, and apart from a little scratchiness in the pots and  
switches it works great. A fun machine and great looks, I think. The  
fast LFO (up to 100 Hz?) was typical from Yamaha's from that era, I  
think.
