Here's the adjusted schematic with the proper transistor placement taken into account:

I just noticed something: When I first tried this, I assumed the inverted triangle supply symbols were ground since the App Note doesn't specify a negative voltage on the schematic. Did they mean -15V instead?

I'll go breadboard this (without the ancillary sections) and post the results.
