Actually, the earliest op-amp handbook/manual was the "Applications 
Manual for Philbrick Octal Plug-In Computing Amplifiers" published in 
1956 by George A. Philbrick Researches, Inc.  Philbrick was the first to 
market a commercial op amp in 1952 (the legendary two-tube K2-W), and 
this book was the first applications manual.  Unfortunately, it's only 
28 pages long.

The follow-up book from Philbrick, "Applications Manual for Computing 
Amplifiers for Modeling, Measuring, Manipulating, and Much Else" was 
published in 1966.  It's still a fascinating read.  You can download it
