the switches are Alpha SR2511F series. List for $2.20 each (quantities less than 50) at Mouser. D-Shaft except where noted.

22 SR2511F-34FN 3 Pole 4 pos. (9 are knurled shaft)
01 SR2511F-26FN 2 Pole 6 pos.
09 SR2511F-43FN 4 Pole 3 pos.

The jacks are Kobiconn 3.5mm mono sockets, Mouser part # 16PJ012. These look to me like the syle of jack that Blacet uses...
