A third (or fourth) type of modulation is the modulation source which is 
depending on the played music. The most wellknown are velocity and 
(don't laugh) keyboard cv.
But I'd really like to see something that produces cv's from 
(time-)differential aspects. E.g.: cv that corresponds to the interval 
to the last note; a cv that corresponds to the difference between this 
notes velocity and the last notes velocity; a voltage that corresponds 
to the count of events per time where the accumulation rate should be cv 
controllable again; feel free to add more...

Also a lot can be done with "envelope followers" which are fed by the 
synthesized signal itself.
