TI (Burr-Brown): OPA451 (+-40V), OPA551 (+-30V), the latter is available 
in DIP8/SO8 if you don't need the full power capability.  Some of the 
older TI "Excalibur" series OpAmps (TLE214x, IIRC) are barely good for 
+-22V supply, too.
