I'm a bit late replying to this, but...

Have you considered the Panasonic ECHU series (available from Farnell)? They 
are the only SMD alternative I've found for the big, close-tolerance PTH 
polystyrenes, etc.

They are SM plastic film parts with a choice of 2% or 5% tolerance. Range of 
values is 100pF up to 220nF, with 0805 or 1206 footprints for values less 
than 47nF.

Soldering is reflow only - don't know if that is a problem for you.

Any good?
