[sdiy] Updated JH Triple Chorus BOM

Hey all,
I've put an updated version of the JH Triple Chorus  BOM with Mouser 
part numbers here:

If you had my old BOM, please replace it with this one.  I did the old 
one off of the component overlay, and several disc caps were listed as 
SMT instead of thru-hole.  One or two resistor values were off, too.

Please let me know if you spot any errors.
