I have two different 4034 and a 4035 in front of me and the differences are
like you described except for the 1k resistor which isn't there.
4035-004REV1 never potted - dual fet 2n3958 - tantalums on powerinput
4034-003 REV-A potted -two fets 2n5459 on the opposite side - metalcan
lm301AH on the opposite side - tempco on opposite side - ceramic caps on
powerinput
the other 4034-003 REV-A potted - 2n3958 - lm301AN opposite side - the
rest looks the same

I get no hits related on that number. That's the only one on these
boards. I had thought they were 4034 or 5's. On closer examination
though....
1) I have a 4034 here. It's absolutely NOT the same. The design of the
board is totally different. Op amp on opposite end etc. The rev. A I
have has discrete FET's. These have a dual FET in a can. But ALSO the
schematic deviates. There is no 1K resistor attached to one of the
input sides but there is on the other. (Pin 12 on the Z2 3086). So I
just wondered before I probe my life away if anyone has a clue what
these things are from. I got half a dozen or so in Tim Smith's stuff.
The first one oscillates at some high requency. Wasn't the output op amp.
But anyway..these are obviously later models because there is no sign
they were potted ever. The 4072 and 75 use the LM3900 with transistor
pairs. Rather than 4 3086's. So I'm not finding a match in any of the
diagrams I can find on the arp submodules page exactly. -Bob
